WellShare's Elders Connections Project promotes community connectedness among Somali elders living in Minneapolis by encouraging their increased use of social and health services and involvement in community programs.

Our community health workers visit elders monthly, lead exercise and discussion classes, and pass out health information at community health fairs to reach elders and keep them active and involved.

This project is made possible by a City of Minneapolis Community Development Block Grant.
